›Are operators exempt from the whitelist?

Yes, operators can always join, whether or not they are listed.

›Does the whitelist use names or UUIDs?

You add players by name, but the server resolves and stores their UUID. That means a whitelisted player keeps access after changing their username, and adding a name that has since been taken by someone else will whitelist the wrong account.
